in lsmw in which case we should create the parent-child structure relation
of source struct. and in which case we should create both the struct. at same level?

In case U have po header Details in one file & po item details is in one file in that case u need to make parent & child relation.
No u should create lower level.

Answer:
Create parent child when the output has to have some data from the parent record (which is not on the child). You use the same level when data off of either of the records should cause an output record.
